Medical Records Release
If you have records at a previous provider, we can request them on your behalf. This is completely optional — you can always provide records later or skip this entirely.
Do you have records at another provider?
If yes, authorizing a release now lets us gather your history before your first visit. If you’re a brand-new patient with no prior records, go ahead and skip.
